Summary: Initiates and provides access to adult agencies’ information and services to transition age students in preparation for exit from educational services. Maintains contact with school and community resources to enhance student employability and transition to adult living in the community. Essential Duties and Responsibilities: SYSTEM CHANGE:
Facilitate the continuous improvement process for YAP in collaboration with principal, teachers, and other staff
Help to drive a focus on outcomes for students around employment
Create natural supports for students
Connect with community agencies
Participate on the KRESA Instructional Rounds Team to support teachers
COMMUNICATION/COLLABORATION:
Participate in weekly meetings with other YAP leaders (principal, school psychologist, behavior specialist, and WBL coordinator) to support action-oriented communication, collaboration, work alignment, meeting preparation, and planning for professional learning
Participate as needed in the weekly case study team meetings to discuss student needs
Support the planning and facilitation of onsite/offsite teacher meetings
Support the planning and facilitation of full staff meetings
Attend KRESA Instructional Leadership Team meetings once a month
Meet with local district and YAP staff to strategically plan for student enrollment to YAP, YCC,and PS programs
Attend IEPs as needed
Facilitate mentor/mentee relationships and their monthly meetings
Develop relationships with local district secondary teachers and be a valuable resource during the transition to YAP
Collaborate withadministration on YAP/YCC tours with parents and local school districts
Collaborate with administration with planning and facilitating on campus events such as New Student Orientation, Open House, and YCC Orientation?
Support the review and development of the enrollment process for the YAP continuumON-SITE SUPPORT:
Facilitate busing at the beginning and end of day and communicate to staff transportation needs
Support Principal with staffing needs as appropriate
Support Behavioral Tech with implementation of Behavior Plans and weekly meetings
Manage and train CPI certification requirements
Attend KRESA ILT meetings
Facilitate continuous improvement process with principal, teachers, staff
BEHAVIORAL SUPPORT:
Coordinate with TC support of students transitioning to YAP (local/KRESA programs)
Support planning/facilitating on campus events
Facilitate busing at the beginning and end of day and communicate to staff urgent busing needs
Support Principal with staffing and behavioral needs as appropriate
Supervise Behavioral Tech position, support implementation of BSP
Must have knowledge of and comply with the policies and procedures contained in the Kalamazoo RESA handbook.Certificates, License, Registration: Michigan Teaching Certificate; Meets requirements for Transition Coordinator as outlined by the Michigan Department of Education’s Office of Special Education and Early Intervention Services Other Qualifications, Skills & Abilities: Embraces and supports KRESA philosophies related to diversity, inclusiveness, and anti-racism.

Travel to offsite locations.Supervisory Responsibilities: This job has supervisory responsibilities.
